questions about A1c test?

Question:
I have been fighting the gestational diabetes test results since they tested me. I finally got them to do an A1C test and it was a 4.9. To my knowledge that means i am on the low end of the normal range. But when I brought that up to the nurse she says it is more complicated then that. Since they are not returning my calls I was hoping you guys can answer this for me.

A1c is an average of 3 months ... so you can be having a perfect A1c but still have high as the only started climbing not long ago and it will not be reflected in the A1c yet
So 105 would be a little bit elevated for fasting but as a non diabetic... your 2 hours after eating should come down to at max 120...
A1c is a good indication for overall control over time... but nothing compares to on the spot testing of to know how high your current is
plus... you can have a good A1c but your is running haywire... going high then going low... in the end it averages out and the A1c will look ok

The 105 is in the prediabetic range and the 151 is high for a normal non diabetic who would be around 100 after eating. The lower HbA1c could indicate that you have a lot of lows along with the highs giving you a "normal" average. I think because you are pregnant they want to make sure they keep your sugars in a healthy range because of the baby.

You asked, so here goes : A good A1C does not necessarilly mean your blood sugar is o.k. I would recommend that you accept your diagnosis and get on board with whatever treatment your healthcare team recommends. Your blood glucose measurements exceed those that are recommended for gestational diabetes and if you do not get them under control, you could be putting your baby and yourself at risk. You can do it - I did, and have a happy healthy 2 year old who had no diabetes related complications.

As hard as it is to take the others are right. The a1c is a delayed reading, it covers the past not the now or future even though its good if your getting the readings you are now its best to do what the medical team says. Eat healthy and cut out as much processed junk food as you can. This will help you for now and in the future good luck to you and the baby.

From what I understand ... the preferred range for pregnant woman is < 7mmol/l (<126mg/dl) at any given time (including after meals) ... obviously for fasting they would want it lower than that... i would assume < 5.5 (100)

Hannah is right - my doc wanted fasting less than 90 and less than 140 an hour after meals, or less that 120 at two hours.

I agree with the other posters... Definately get on board with your diagnosis... Gestational diabetes is caused by the placenta... So is most cases you will return to normal after the baby is born.
My doctor let me go undiagnosed with GD... My son was born premature because of that.... He was a whoppping 6 lbs 9 oz at 33 weeks gestation, and had a heart murmur, hadn't developed the sucking reflex yet so he had to be tube fed, and stopped breathing at one point... On top of all of that I almost died...
So please do as your doctor tells you.

As great as the A1c is, the glucose tolerance test is still the best test to diagnose diabetes - gestational or otherwise. The good news, is that since the A1c is normal, you are virtually certain to be early in the course of diabetes. Since it started in pregnancy, you will probably go back to normal glucose tolerance after you deliver. During pregnancy, blood sugars need to be as close to non-diabetic as possible for a healthy mom and baby. (Whereas in non-gestational diabetes we say 140 2-3x a week is no problem, in gestational diabetes, we say what can we do to get it under that.) In any case, even when (not if, because we all know you're gonna have a great pregnancy, and be as healthy as possible, right?) your 's are controlled, you still need to be screened for type 2 diabetes at your 6 week postpartum visit, and anually for at least a few years after that, simply because gestational diabetes puts you at risk for type 2 later in life. Sorry if I'm scaring you. Just remember, this is a great place for answers, esp. if your doc isn't able/willing to answer them for you.

no insulin Gestational diabetes can hit in a flash. 19 years ago, I passed my at 12 weeks pregnancy and went on to deliver a 10 1/2 pound baby 24 weeks later! You can go from slightly impared to full blown in a matter of months. THAT said, once you learn how to count the amount of carbs you eat (you most likely meet with a nutritionist & be given a meter to track your blood sugars), control will be a snap & will do just fine. I eventually developed Type2 that is here to stay.
